Admissions

Admitting can go smoothly if we work together. From you we will need:

• Medical cards
• A copy of your Health & Physical from your doctor
• Plan of care, or discharge orders from your doctor and/or hospital

These items can be faxed to (408) 245-2433. Please write "Attention: Admissions" on the cover page.

In advance

There are several documents that need signing, and completing them ahead of your admission will ensure a more relaxed admission day. Please contact our admissions department at (408) 245-8070 to schedule an appointment to complete an admissions packet. This is especially important if you have a surgery date scheduled, so that we can welcome you immediately.

Billing

For Medicare, Medi-Cal and medical insurance as primary payer sources, ther are no upfront fees to be paid. If you plan on paying privately, your first month's payment will be due on admission, and you will be billed at the beginning of each month thereafter. If you do not stay an entire month, the balance of the month, minus any ancillary charges, will be refunded.
